The artist said that he made this head based on his studies of an old, poor Scottish woman. The subject matter and the realistic modelling of the aged skin reflect New Sculpture principles. He also made another sculpture, 'Head of a Baby', to go with this piece. Lever bought that work in 1916 but its location is unknown.
